1. Prepayment when appointment is scheduled – 5% Fee Reduction
2. Payment in full at time of appointment
3. 6 Month No Interest financing through CareCredit – 5% Administrative Fee ( $50.00 minimum)
4. 12 to18 Months No Interest financing through CareCredit – 7.5% Administrative fee ($100.00 minimum)
      For 18 Months minimal financing of $3,500.

We will submit claims to your insurance carrier and apply any benefits they allow against your account. Any balances not covered will be the responsibility of the patient.
